”If you take a closer look at the last episode, you will see that everything is quite clear” said the series creator David Chase, who had conceived an idea for such ending two years before the actual end of the show.
According to Chase’s statement, we can conclude that Tony Soprano is most likely dead, that he was killed in the last scene where he was having dinner with his family at a restaurant. Also there is the question: who killed Tony and why? We will try to clarify this.
1) The opening sequence of the last scene shows Tony’s point of view (POV). Whenever someone enters the restaurant and the doorbell rings, we see Tony’s reaction, and then from his perspective we watch who is entering the restaurant.

The explanation of this can be found in the episode when Tony and Bobby Bacala are sitting in a boat, fishing and talking about death, when Bobby says to Tony: “You probably won’t even hear it when it happens, right?”

Remember how Gerry Torciano was killed in the restaurant while he and Silvio were having dinner with their mistresses. Silvio didn’t hear or predict Torciano’s murder.
First he felt drops of blood on his clothes and face, and then he heard a gunshot. Later, during his conversation with Tony, Silvio said: “Fucking terrible thing, I didn’t know what was going on until after the shot was fired. Fucking weird.”

In the final episode, Phil Leotardo was killed in the same way. Just like Tony, Phil could not hear the bullet that was fired into his head and he was also killed in the presence of his family. You can also notice Phil’s wife’s delayed reaction to him being murdered:
Tony Soprano’s murderer was the man in a jacket sitting at the bar. The man (probably a professional killer) had looked at Tony’s direction since entering the restaurant, and then, having identified his target (Tony Soprano), he passed by Tony’s table and went to the bathroom, from where he could freely shoot Tony at the back of his head.
The reason why Tony was killed was most probably revenge for the humiliating death of the New York mafia boss Phil Leotardo (who was killed in the presence of his wife and grandchildren, and whose head was run over by a vehicle after the murder). The murder could not go unpunished. That was why Tony had to be killed.

The man in a jacket is entering the toilet. Later, after leaving the toilet, he had an open space to shoot Tony in the head.

According to the scenes from the last episode and the series creator David Chase’s statements (who has never openly said what happened to Tony), we can conclude that Tony was killed!
Although it is unlikely, maybe the authors, producers and actors decide to shoot a movie and make some more money, so Tony Soprano survives the family dinner at the restaurant.
